Medication History technician based in the Emergency Department.

The Certified Pharmacy Technician works under the lead supervision of a Pharmacist to provide services to Pharmacy customers. The Pharmacy Technician supports the Pharmacist in providing Pharmaceutical Care.
identify patients appropriate for medication history intervention based on defined criteria and collaboration with nursing staff
interview patients, as well as family/caregivers as needed, to accurately gather a current list of medications
contact outside facilities (skilled nursing facilities, retail pharmacies, physician offices) as needed to ensure medication history is complete
report to hospital pharmacist any discrepancies, duplicate therapies, or discontinued medications and any barriers to adherence
document medication history in EPIC
Minimum Qualifications Required
Education:A high school diploma or equivalent is required. Successful completion of an approved Pharmacy Technician training course is preferred. Successful completion of Pharmacy Service Line competencies within 6 months of hire.
Experience:Twelve (12) months hospital clinical experience of two (2) years retail pharmacy experience preferred.
Licensure, certification, and/or registration:Registration with the Virginia Department of Health Professionals required. Pharmacy Technician Certification Board (PTCB) certification is required within 9 months of hire.
Other Minimum Qualifications:Must be able to maintain familiarity with a large number (1000-2000) of medications and focused on attention to detail/accuracy. Must be customer service oriented with strong verbal communication skills, as well as attention to detail. Requires knowledge and use of general office equipment and procedures, including computer/keyboard. Must be able to solve simple mathematical and logistical problems in the work environment. Must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision. All Technicians must complete Orientation Competencies.
